Electronic Input of Hospital & Activities Reporting

VFW Posts through-out the state of Michigan can now enter their Hospital and Activities reports on-line.  After the reports are approved...the report becomes viewable on the Reports and Forms page. 

Currently the Department every month mails out two packets to every post of the same items we make printable on our website...so with your help by applying below you and your VFW Post will help us to cut down the high cost of mailing these packets and also save your VFW Posts the cost of postage to mail these copies back to our Department. 

If your VFW Post has not signed up for this new reporting yet....please go down to the bottom of this page and fill out our application form...Because together we can help move the Department of Michigan Veterans of Foreign Wars into the 21st Century...and with your help...We Will Succeed!

        Special Note:  When using these forms - please make sure your
                                 Dates are in the format such as:  02/01/2006
                                 on both forms or your reports will not be sent to
                                 our data base for approvals. 

                                * To use these forms correctly... Internet Explorer must be the Browser  you are using...
                                   programs i.e.  "Fire Fox" will not work with this system.....
