2008– 2009 ACTIVITY EVALUATION PROGRAM
Please read this carefully and pass it on to your chairmen.
Use the form titled "Department of Michigan VFW Program Report Form". Do not use the old National report forms.
Reporting dates for this year will be from May 1, 2008 to April 30, 2009. Only projects completed after May 1, 2008 will be credited for the 2008-2009 program year. Reports received in Department by the 10th of each month will be credited on the end of month Department Activity Report. Make sure you fill out all information required. Money value should use the mileage rate of $0.14 per mile per vehicle, actual money spent, value of hall rental, and value of items donated. Do not include value of volunteer hours, this is not acceptable according to the IRS.
List each project only once on the Program Report Form. Credit will be given on the Department Activity Report in each program in which credit is earned for each project reported.
Do not report any Auxiliary activities on your Post activity report. Auxiliary report information will be obtained directly from the Department Auxiliary Headquarters.
Do not list hospital activities on the program report form. You will receive credit on the Department Hospital Report based on the information provided by the Post on the Department Hospital Report Form. Do not include Cancer or PAC donations on the Department Activity Report Form. The Department Quartermaster and the PAC Director based on the actual contributions received will credit those donations on the Department Activity Report.
Send two copies of any letters to and from elected officials along with your report for Legislative credit to Department Headquarters. One copy for the legislators file the second for Activity Report.
Make sure we can read your report. Make sure you identify your Post and District and sign the report. Send only one copy of the completed Program Report Form to Doug Masseau, Director at the address below. Retain one copy for Post records.
Posts who do not already report the activities online are encouraged to do so. Contact the Webmaster Gary Brown through the Department website: www.vfwmi.org. He will assign you a password for filing your Post reports.
Districts and County Councils are not required to submit reports.
Please review the All-State Program for maximum credits. SUBMIT YOUR REPORTS ONCE PER MONTH. REPORTS ARE DUE BY THE 10TH DAY OF EACH MONTH FOLLOWING THE ACTIVITY. DO NOT MAIL YOUR REPORTS WEEKLY. YOU WILL ONLY RECEIVE HALF CREDIT FOR ACTIVITES REPORTED NINTY (90) DAYS AFTER THE ACTIVITY IS COMPLETED. Let's have another great year of reporting. PAC donation (Credit given by PAC Director). Cancer donation (Credit given by Department Adj/QM). Hospital donations & blood drives (Credit on Hospital Report). Nursing Homes and volunteer work (Credit on Hospital Report). Children’s Hospital donation & work (Credit on Hospital Report). Clean up and other on VFW property. Attending VFW meetings – Post, County Council, District, July C of A, POW-WOW, Mid-Winter and Department. Saying the Pledge of Allegiance or Posting the flag at Post events. Donating Post home for VFW meetings, County Council, District, etc. District or County Council Commanders visiting other Post meetings.
